Medicaid Waiver Case Management Services
Benefis Community Care case management teams are responsible for determining the needs of 1,500 members. They coordinate services to ensure that member needs are properly met while maintaining fiscal responsibility for the state’s budget.
Benefis Community Care’s case management services allows 1,500 Montanans – who would otherwise be institutionalized – to live in their own homes and communities.
Benefis Community Care entered into a partnership with the State of Montana Department of Public Health and Human Resources Senior and Long-Term Care Division in 1998 to provide case management services to the elderly and people with physical disabilities under the Big Sky Waiver Program in Gallatin and Silver Bow counties. The service areas for Benefis Community Care have expanded over the years, and Benefis Community Care was awarded the Big Sky Waiver Case Management contract in 2018 to serve 53 of the 56 Montana counties.
