Ways to Access Your Medical Records
If you would like to receive access to or a copy of your medical record, please sign up for our patient portal, MyChart, or send us a request for your medical records.
The walk-in office at Medical Records located on Benefis East Campus is open for on-site records requests and pickup services from 8 a.m. to 5 p.m. Monday through Friday.
If you would like for us to send a copy of your medical record to a third party, such as a lawyer or family member, please download and complete our "Authorization to Release Medical Information" form below. Questions can be directed to our main telephone number at (406) 455-5642.
IMPORTANT: Please complete the authorization form in its entirety. You must send a copy of your valid driver's license, passport, or other government/legal form of identification. This is required to properly identify you and protect your privacy.

Options for sending us your request or completed authorization and valid ID:

- Email: medical_records@benefis.org Drop the completed documents in the secure/locked box located outside of Medical Records in Benefis North Tower.
- Call (406) 455-5642 to schedule an appointment.
- Walk in at your convenience during normal business hours.
Please note: The fastest way to obtain your medical record is through signing up for our patient portals, or faxing the request to us. Under HIPAA, your provider may obtain medical records from us without an authorization and you can request that they contact us to request the records.
